When would you use this form?
Suppose you have a death certificate and gives the date of death. Suppose it does not give the date of birth but instead gives the age rather precisely -- so many years, so many months, and so many days. You would enter the date of death for the second event and the precise age for the elapsed time. The first event would then give the date of birth.
