Accessing the Illinois Death Index in One Step

On 11/15/2019 my server became blocked from accessing the Illinois death records.

You can search those records directly from Illinois State Website pre 1916 and 1916-1950
but you will not be able to take advantage of all the added features that my form offered.

 For pre-1916 records
     there are no required fields, not even the last name
     the middle name field is treated as starts-with
For 1916-1950 records
     at least the first letter of the last name is required
     all names are treated as starts-with
     the volume and page fields are ignored
